A renewal inspection was conducted on September 12, 2023 from 2:10 pm to 4:20 pm. The provider and assistant were available during the inspection. There were seven children present, ranging from 13 months to 5 years of age (total of 15 points) with three caregivers supervising. A total of five children records and three caregiver records were reviewed. A walkthrough of the entire home was conducted.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 8VAC20-800-180-A Description: Based on review of three caregiver records, the provider did not obtain a current report of Tuberculosis Screening every two years from the date of the first screening for two caregivers.

The record for caregiver #1 documented a tuberculosis screening dated 8/20/21.

Standard #: 8VAC20-800-460-C Description: Based on documentation reviewed and interview, the provider failed to ensure all household pets (dogs or cats) are vaccinated for rabies and treated for fleas, ticks or worms as needed.
Evidence:
1. During the inspection, one dog was observed in the provider?s home and during an interview, the provider acknowledged having two dogs.
2. The provider was not able to locate documentation of the two pets? vaccinations.
