Comments:
An unannounced monitoring inspection was conducted on 8/31/23 from 8:52a.m. to 10:23 a.m. with the provider and two assistants. There were 11 children in care, ranging in age from 16 months to 4-years-old, for a total of 25 points. Children were observed eating breakfast, circle time, singing, dancing and outside play. Four child records, and three caregiver records were reviewed. The provider has current certification in CPR and First Aid. No medication was on site and the provider has her certification in Medication Administration Training (MAT) Breakfast served was cereal, milk and bananas. The first aid kit, flashlight, weather band radio, and fire extinguisher were observed. The emergency drill log, attendance, and menu were also reviewed. Areas of non-compliance are identified in the Violation Notice. Based on review of three caregiver records, the provider did not obtain documentation of the results of a central registry search within 30 days of employment for all staff members.

Evidence: The record for caregiver #2, with a start date of 1/3/23, did not contain documentation of the results of a central registry search at the time of inspection.

Standard #: 8VAC20-800-430-B
Description: Based on observation the provider did not ensure that accordion expansion gates were not used as protective barriers at stair openings.

Evidence: A large metal accordion expansion gate, approximately 6 feet in width, was located at the top of the concrete stairs that lead down from the playground area into the residence. At the time of inspection a child placed their head in one of the diamond shaped openings.
