Technical Assistance:
Technical assistance the provider is not operating a Family Day Home which requires she operates under the parameters.

Comments:
An unannounced monitoring inspection was conducted September 19, 2023 approximately 1:55 pm to 2:40 pm. There were 7 children for a total of 14 points directly supervised by 2 staff. The home, 3 staff records, 3 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on-site today.

Children were observed resting. There was an assortment of age appropriate materials for children. The home was clean and organized.

Violations:
Standard #: 8VAC20-800-30-A
Description: Based on observation of the family day home, the provider did not ensure compliance with these standards and the terms of the current license issued by the department and with relevant federal, state or local laws, and other relevant regulations.
Evidence:
1. Provider does not currently meet the definition of a Family Day Home as she does not reside in the house. This was determined when the Licensing Inspector conducted a walk-through with the assistant caregiver of the home located at 8809 Arlington Boulevard, Fairfax, VA and found no furniture, personal supplies in the bathrooms or clothes in the closet.

Plan of Correction: Currently in the process for CDC facility made building changes over the summer.

Standard #: 8VAC20-800-180-A
Description: Based on staff records review, the provider did not obtain for each caregiver a current Report of Tuberculosis Screening form, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
1. Staff #2 Tuberculosis Screening form expired on 7/31/2019

Plan of Correction: Will complete TB test by the end of Oct.
