Comments:
An unannounced monitoring inspection was conducted today between 10:30am and 11:50am. There were 11 children present with the Provider and 2 assistants. The children were observed participating in circle time, eating a morning snack (banana slices, apples and mandarin oranges) while watching cartoons and playing freely with blocks and cars. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard #: 22.1-289.035-B-2
Description: Based on record review and interview, the provider has not obtained fingerprint results for each caregiver prior to working with children.
Evidence: Caregiver #3s record does not contain fingerprint results. The provider confirmed that caregiver #3 has not yet completed fingerprinting.

Plan of Correction: An appointment has been made for later this week.

Standard #: 8VAC20-770-60-B
Description: Based on record review and interview, each employee has not completed a sworn disclosure statement.
Evidence: Caregiver #3s record does not contain a completed sworn disclosure statement. The provider confirmed that caregiver #3 has not signed a sworn disclosure statement.

Plan of Correction: They can do it today.

Standard #: 8VAC20-800-730-D
Description: Based on observation, medication was not kept in a locked place using a safe locking method that prevents access by children.
Evidence: An EpiPen was observed in the pocket of a backpack on the bottom cubby of the childrens storage cubbies. The EpiPen was not locked.

Plan of Correction: We will move it.

Standard #: 8VAC20-800-750-A
Description: Based on observation topical skin products were not labeled with a child's name.
Evidence: Two diaper creams were observed in the diaper changing area without names to identity the owners.

Plan of Correction: Expired creams were removed.

Standard #: 8VAC20-800-750-D
Description: Based on observation topical skin products were kept beyond the expiration date.
Evidence:
1. A and D cream was observed that expired 3/2019.
2. Desitin cream was observed that expired 6/2021.

Plan of Correction: We will label them.

Standard #: 8VAC20-800-830-C
Description: Based on record review and interview, documentation of drills was not maintained.
Evidence: There is no evacuation drill documented for the month of September. The provider stated that they did a drill but didn't document it.

Plan of Correction: I will make sure we write it down each month.

Standard #: 8VAC20-800-920
Description: Based on observation, menu requirements were not met.
Evidence: There is no menu posted.

Plan of Correction: I'm working on putting together a new menu. I will post it today.

Standard #: 8VAC20-800-120-B
Description: Based on record review, assistant caregivers records did not contain all required information.
Evidence: Caregiver #3s record does not contain documentation of two references, contact information of someone to be contacted in case of an emergency, documentation of education, background checks as required by 8VAC20-770 and tuberculosis results as required by 8VAC20-800-170.

Plan of Correction: She just started today. I can get the record completed today.

Standard #: 8VAC20-800-60-B
Description: Based on record review, childrens records did not contain all required information.
Evidence: Child #9s record does not contain addresses for both emergency contacts.

Plan of Correction: I can get it today.

Standard #: 8VAC20-800-170-A
Description: Based on interview, the provider did not obtain a current report of tuberculosis (TB) screening for each employee prior to coming into contact with children.
Evidence: The provider confirmed that caregiver #3 has not provided a TB screening. Caregiver #3 was observed working during todays inspection.

Plan of Correction: They have an appointment scheduled later this week.

Standard #: 8VAC20-800-690-D
Description: Based on observation, the diaper chancing surface was not non-absorbent.
Evidence: There were two small tears observed in the diaper changing mat.

Plan of Correction: Replaced during the inspection.
