Comments:

An unannounced renewal inspection was conducted on 10/10/23 from 12:40pm to 1:45pm with the provider and two assistants. There were five children in care, ranging in age from four-months to two-years-old, for a total of 17 points. Children were observed napping and beginning to wake. Five child records, two caregiver records, and two household member records were reviewed. The provider has current certification in CPR and First Aid. The provider does not give medication and understands that they must obtain certification in Medication Administration Training (MAT) in order to give medication. The first aid kit, flashlight, weather-band radio, carbon monoxide detector, and fire extinguisher were observed. The emergency drill log, attendance, menu, and required postings were also reviewed. Description:	Based on review of documentation and interview, the provider did not obtain the results of a national, fingerprint-based criminal history record check for each caregiver prior to employment. Evidence: On 10/10/23 Caregiver #3 was present with the children and their name was documented as an assistant in child records. Interview revealed that Caregiver #3 had started working on a part-time basis in approximately September 2023. There were no fingerprint results on record for Caregiver #3.
Plan of Correction:	Fingerprint -based criminal history record check was obtained for caregiver 3.
Standard #:	8VAC20-770-60-B
Description:	Based on review of documentation and interview, the provider did not obtain a sworn disclosure statement from each caregiver prior to employment. Evidence: On 10/10/23 Caregiver #3 was present with the children and their name was documented as an assistant in child records. Interview revealed Caregiver #3 had started working on a part-time basis in approximately September 2023. There was no sworn disclosure on record for Caregiver #3.
Plan of Correction:	Sworn disclosure statement was obtained for caregiver 3 on 10/16/23.

Description:	Based on review of documentation and interview, the provider did not maintain a record for each caregiver. Evidence: On 10/10/23 Caregiver #3 was present with the children and their name was documented as an assistant in child records. Interview revealed that Caregiver #3 started working on a part-time basis in approximately September 2023. There was no record available for Caregiver #3.

Standard #:	8VAC20-800-170-A
Description:	Based on review of documentation and interview, the provider did not obtain a current Report of Tuberculosis Screening form (TB screening) from each caregiver at the time of hire and prior to coming into contact with children. Evidence: On 10/10/23 Caregiver #3 was present with the children and their name was documented as an assistant in child records. Interview revealed that Caregiver #3 had started working on a part-time basis in approximately September 2023. There was no TB screening on record for Caregiver #3.
