Comments:
An unannounced monitoring inspection was conducted on 10/11/23 from 2:55pm to 4:10pm with the provider. There were 12 children in care, ranging in age from six-months to four-years-old, for a total of 32 points. Children were observed waking from nap, using the restroom/having diapers changed, going outside, and eating a snack. Two new child records, three caregiver records, and two household member records were reviewed. All three caregivers have current certification in CPR and First Aid. The provider reported that they do not give medication and understands that they must obtain re-certification in Medication Administration Training (MAT) in order to give medication. Snack served was veggie straws and milk. The emergency drill log, attendance, and menu were reviewed. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 10/20/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-800-90-A
Description: Based on review of two child records, the provider did not obtain documentation that a child had been adequately immunized according to the requirements of the Code of Virginia and applicable State Board of Health regulations before the child attended the family day home. Evidence: The immunizations on record for Child B, with a start date of 8/7/23, were dated 9/15/23.

Plan of Correction: I will make sure to request a hard copy of immunization records instead of an electronic one before start date.

Standard #: 8VAC20-800-180-A
Description: Based on review of three caregiver records, the provider did not obtain a current Report of Tuberculosis (TB) Screening form, every two years from the date of the first screening for each caregiver. Evidence: The most recent TB screenings on record for Caregiver #1 and Caregiver #2 were dated 9/2/21. The most recent TB screening on record for Caregiver #3 was dated 9/1/21.

Plan of Correction: Will make sure to update all TB records a month in advance. Screening forms will be update this week.

Standard #: 8VAC20-800-690-E
Description: Based on observation, soiled disposable diapers and wipes were not disposed of in a leak-proof or plastic- lined storage system that is either foot-operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal. Evidence: The licensing inspector observed soiled disposable diapers/pull-ups and wipes in the open bathroom trashcan, and the trashcan did not contain a lid and was not foot-operated.

Plan of Correction: Trashcan has been replace for one with lid and foot operated
