Comments:

An unannounced monitoring inspection was conducted on 10/17/23 from 10:40amto 11:40am At the time of the inspection there were 5 children in care and one caregiver. Children were observed eating lunch, participating in floor play and preparing for nap. No medications are administered by the provider. The Inspector reviewed 5 children?s records and 3 caregiver and household member records. Required postings, emergency drills and emergency supplies were also reviewed. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Description:	Based on record review and interview, it was determined the provider did not ensure that documentation of a physical examination by or under the direction of a physician prior to a child's attendance is obtained prior to the child/s attendance or within 30 days after the first day of attendance. Evidence: 1. The record provided for child #2 indicated an attendance start date of 9/5/2023. Documentation of a physical examination was not in the child's file. 2. The provider verified that she did not have documentation of the child's physical examination.
Plan of Correction:	The provider responded, the parent will be notified today that a copy of the physical examination must be submitted.
