Standard #: 8VAC20-800-920 Description: Based on document review, the current one-week period menu did not meet all requirements.
Evidence:
1) The menu was not dated as required. 2) The menu did not list substitutions as required. Today's breakfast consisted of pancakes, apple slices and milk. The menu had listed bananas as the fruit to be served. The substitution was not listed.Plan of Correction: Provider dated the menu. Provider also listed all the substituted food/fruit on the menu.
Standard #: 8VAC20-800-960-B Description: Based on observation, the provider did not ensure that infants who cannot hold their own bottles be picked up and held for bottle feeding and that bottles were not propped.
Evidence:
1) On the date of inspection, the provider placed an infant (6 months old) in a crib and propped a milk bottle with blankets for him to drink.Plan of Correction: Provider holds the bottle for the baby to feed him.
Standard #: 8VAC20-800-960-C Description: Based on observation, safety waist and crotch straps were not fastened according to the manufacturer's instructions in high chairs used by children.
Evidence:
1) Four children were seated on high chairs without the safety waist and crotch straps being securely fastened.Plan of Correction: Provider fastened all the kids safety belt of high chairs.
Standard #: 8VAC20-800-100-A Description: Based on record review, provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1) The record for Child #1 (start date 08/21/2022) did not contain documentation of a physical examination.Plan of Correction: The child's parents provided the documentation.
Standard #: 8VAC20-800-50-A Description: Based on document review, a written record of children in attendance each day was not kept.
Evidence:
1) At the time of entrance, there were four children in care with the provider. The record of attendance did not have children recorded in attendance for the day.Plan of Correction: Provider will make sure to write the attendance time/date when the kids come.
Standard #: 8VAC20-800-60-B Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #3 did not contain the Provisions of the Emergency Plan.Plan of Correction: Provider asked the parents to sign the Provisions of the Emergency Plan.
Standard #: 8VAC20-800-90-C Description: Based on record review, additional immunizations were not obtained once every six months for children under the age of two years.
Evidence:
1) The most recent immunizations in the record for Child #1 (dob 09/20/2021) were dated 10/06/2022. Updated immunizations are due for Child #1.Plan of Correction: The parent will update the immunization record.
Standard #: 8VAC20-800-320-B Description: Based on observation, protective covers were not installed on all unused electrical outlets accessible to children under five years of age.
Evidence:
1) An electric outlet was observed not in use and without a protective cover.Plan of Correction: All the outlets have protective covers.
