Technical Assistance:
Discussed bottle/crib labeling procedures and records.

Comments:
An unannounced monitoring inspection was conducted today between 12:15pm-1:45pm. There were 6 children present with the Provider and 2 assistants for a total of 18 points within the supervision guidelines. The children were observed drinking a bottle, having a snack, taking a nap, and playing with toys. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, evacuation drill log, attendance and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation. Standard #: 8VAC20-800-60-B
Description: Based on a review of records, 1 out of 3 child records did not contain the required information.

Evidence:
1. Child #1 (doe: 10/30/2023) did not have documentation of name, phone number, and address of 2 out of 2 required emergency contacts at the time of the inspection.

Plan of Correction: We will get emergency contacts from the parent as soon as possible.

Standard #: 8VAC20-800-90-A
Description: Based on a review of child records, 1 out of 3 children did not have the required documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.

Evidence:
1. Child #1 (doe: 10/30/2023) did not have documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations at the time of the inspection.

Plan of Correction: We will get a copy of the immunizations from the parent as soon as possible.

Standard #: 8VAC20-800-180-A
Description: Based on a review of records, all caregivers and adult household members did not have a current Report of Tuberculosis Screening conducted every two years from the date of the first screening.

Evidence:
1. Caregiver #3's (doh: 1/20/2022) last tuberculosis screening on file at the time of the inspection was dated 9/22/2021.
2. Household Member #1's last tuberculosis screening on file at the time of the inspection was dated 9/22/2021.

Plan of Correction: They scheduled their TB screening and have received results now.
