Technical Assistance:

Discussion of the Parent Handbook versus using the Information for Parents form. FDH includes all information required in handbook. A signed review of handbook is kept on file for each child. Previously cited violation for the Initial inspection dated - 08/15/2023 was corrected. Paperwork for an inspection on the home's fireplace and chimney was reviewed. This documentation was obtained on 09/20/2023.

Comments:

This non-mandated, follow-up monitoring inspection was initiated by licensing staff and concluded on 11/13/2023, between the hours of 1:30 pm to 2:30 pm. There was 1 toddler-age child present (3 points), with 1 supervising. The child was engaged in the following: naptime, diaper changing, hand washing, snack, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication (Provider obtain MAT certification); special care and emergencies; and nutrition. A total of 1 child's records, 1 staff record, and 3 household records were reviewed. Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home. Description:	Based on observation, the emergency numbers were not posted. Evidence - 1. On the date of inspection (11/13/2023) the following numbers were not posted: 911, and the number for the regional poison control center. 2. This was confirmed on the date of inspection by the Provider.
Plan of Correction:	These numbers will be posted on the FDH bulletin board, beside the telephone.

Description:	Based on review, 1 child's file indicated that not all of the required documentation was contain within their file. Evidence - 1. On the date of inspection (11/13/2023), the following documentation was not available for review for Child #1: the Medication Administration - Decision to Administer form; and the Provisions of the Emergency Preparedness and Response Plan form. 2. The start date for Child #1 was listed as - 10/30/2023.
