Comments:
An unannounced renewal inspection was conducted on 12/11/2023. There were 9 children, ages 9 months- 3 years, totaling 23 points, one caregiver and one household member present during the inspection. Two inspectors arrived for the inspection at approximately 10:15 am and departed at approximately 11:40 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

After receiving plans of correction from the provider, the inspection documents were updated on 12/26/2023 to reflect the plans of correction.

Violations:
Standard #: 8VAC20-800-100-A
Description: REPEAT VIOLATION
Based on record review, the provider failed to ensure that prior to a child?s attendance or within 30 days after the first day of attendance, documentation of a physical examination by or under the direction of a physician shall be obtained.

Evidence:
1. The record for Child 1 contained a documented start date of 05/03/2023; The record contained a physical examination dated 10/17/2023.
2. The record for Child 2 contained a documented start date of 11/16/2022; the record did not contain documentation of a physical examination.

Plan of Correction: I will make sure that I have all paperwork before enrolling a child.

Standard #: 8VAC20-800-180-A
Description: Based on record review and interviews with caregivers, the provider failed to obtain for each caregiver and adult household member a current report of Tuberculosis Screening form every two years from the date of the first screening or more frequently as recommended by a physician or local health department.

Evidence:
1. The most recent Tuberculosis Screening for Caregiver 1 was dated 11/16/2021. Caregiver 1 confirmed there was no 2023 screening for review during the inspection.
2. The most recent Tuberculosis Screening for Household Member 1 was dated 11/16/2021. Caregiver 1 confirmed there was no 2023 screening for review during the inspection.

Plan of Correction: We both have appointments on 12/26/2023.

Standard #: 8VAC20-800-210-A
Description: Based on record review and interviews with caregivers, the provider failed to ensure that caregivers shall obtain a minimum of 16 clock hours of training annually in areas relevant to their job responsibilities.

Evidence: Caregiver 2 had 1.5 hours of annual training documented. Caregiver 1 confirmed the required 16 hours of training was not obtained.

Plan of Correction: She is in the process of getting training online.

Standard #: 8VAC20-800-460-C
Description: Based on record review, the provider failed to ensure that dogs or cats, where allowed, shall be vaccinated for rabies and shall be treated for fleas, ticks, or worms as needed.

Evidence: The application for renewal indicates a K-9 that lives in the home or on the premises. Documentation for rabies vaccination was not available for review.

Plan of Correction: He will be going to the vet on 12/26/2023.

Standard #: 8VAC20-800-570-A
Description: FOURTH REPEAT VIOLATION
Based on observation, and record review, the provider failed to ensure that a caregiver does not exceed 16 points, determined by the point system in the standards, for each caregiver.

Evidence:
1. There were 9 children, totaling 23 points, present during the inspection, requiring a total of two caregivers. There was one caregiver present during the inspection.
2. The provider was previously found exceeding the number of points for one caregiver during inspections on 11/20/23, 7/17/2023, 06/30/2023 and 06/22/2023.

Plan of Correction: I'm in the process of getting more help.
