Comments:
An unannounced renewal inspection was conducted today between 11:15am and 12:50pm. There were 8 children present with the Provider and 1 assistant for a total of 24 points within the supervision guidelines. The children were observed playing with trains, listening to holiday music and eating lunch. Diaper changing procedures were observed. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations:
Standard #: 8VAC20-800-100-A
Description: Repeat Violation.
Based on record review, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #1 (date of enrollment: 3/21/2021) record does not contain a physical exam.
2. Child #8 (date of enrollment: 9/5/2023) record does not contain a physical exam.

Plan of Correction: I will have the parents bring it to me this week.

Standard #: 8VAC20-800-60-B
Description: Repeat Violation.
Based on record review, children's records did not contain all required information.
Evidence:
1. Child #3s record does not contain the information for parents form.
2. Child #5s record does not contain a second emergency contact.
3. Child #7s record does not contain the information for parents form, verification of the providers emergency plan and insurance liability declaration.

Plan of Correction: I will have the parents give me the missing information this week.

Standard #: 8VAC20-800-320-B
Description: Based on observation, child-resistant protective covers were not installed on all unused electrical outlets and surge protectors accessible to children under five years of age.
Evidence: An unused outlet in the infant nap room, under the window, did not have a protective cover.

Plan of Correction: Corrected during the inspection.

Standard #: 8VAC20-800-680-A
Description: Based on observation, caregiver hand washing requirements were not met.
Evidence: The provider was observed changing four children's diapers. The provider did not wash their hands at the end of each diaper change.

Plan of Correction: I will have to switch the play rooms around so that I have access to the bathroom.

Standard #: 8VAC20-800-680-B
Description: Based on observation, children's hand washing requirements were not met.
Evidence: Four children were observed having their diapers change. Four out of four children's hands were not washed after their diaper change.

Plan of Correction: I will have to switch the play rooms around so that I have access to the bathroom.
