Comments:
An unannounced non-mandated inspection was conducted on 8/30/2023 from 11:50am-2:15pm. There were 6 children directly supervised by the caregiver and one assistant. The home, 3 children?s records, evacuation drills, and emergency supplies were inspected. Children were observed playing freely and eating lunch. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-800-60-B
Description: Based on record review, 3 of 3 children's records did not contain all required information.

Evidence:
1. The record for Child #1, 2, and 3 did not contain documentation of the first date of attendance.

Plan of Correction: 1st date of attendance will be added to the record.

Standard #: 8VAC20-800-70-A
Description: Based on record review, parents were not provided all required information prior to the first day of attendance for 3 of 3 children.

Evidence:
1. The records for Child #1, 2, and 3 did not contain documentation indicating whether or not there is liability insurance of at least $100,000 per occurrence
and $300,000 aggregate in force on the family day home operation.

Plan of Correction: The information will be provided to the parent and we will get signatures.

Standard #: 8VAC20-800-90-A
Description: Based on record review, the provider did not obtain documentation that one child has been adequately immunized before the child's first day.

Evidence:
1. Child #2 began attending the Family day home on 7/31/23. The immunization documentation was dated 8/15/2023.

Plan of Correction: We will get immunizations before their first day.
