Technical Assistance:
Discussed obtaining medication administration training for the emergency medication currently on site. Medication may only be administer with a completed authorization form from the child's parents in addition to the training certification.

Emailed the provider a copy of a fillable medication policy form to be filled out by the provider and signed by the parents.

Comments:
An unannounced monitoring inspection was conducted today from 9:00am-10:00am. There were 4 children directly supervised by 2 caregivers. The child care area, 2 caregiver records, 4 children?s records, 1 household member record, evacuation drills, emergency supplies, and policies were inspected. There was one medication on site today, however the provider and assistant are not currently authorized to administer the medication. Children were observed playing freely, reading and napping. There was an assortment of age appropriate materials for children. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22.1-289.011-F
Description: Based on observation, all documents required by the Superintendent were not posted in a conspicuous place on the licensed premises.
Evidence:
The findings from the most recent licensing inspection were not posted in the facility.

Plan of Correction: I will post all required
documents on the bulletin
board.

Standard #: 8VAC20-800-60-B
Description: Based on record review, children's records did not contain all required information.
Evidence:
1. Child 3 and 4's record did not contain 1/2 required emergency contacts.

Plan of Correction: I will speak to the parents
and get required information.

Standard #: 8VAC20-800-70-A
Description: Based on record review, the provider failed to inform parents in writing of all required information.
Evidence:
1. Child 1 and 4's record did not contain documentation that parents were informed of the emergency evacuation location.
2. Child 2, 3, and 4's record did not contain all required information for the providers medication administration policy.
3. Child 1's record did not contain documentation that they have been informed of the provider's liability insurance status.

Plan of Correction: 1. Last year the information
was posted on the door and all parents
took a picture for reference.
2. We will updates as soon
as possible.

Description: Based on observation, furnishings in the family day home me were not maintained in a safe and clean condition.
The varnish on the railings of the cribs is peeling.

Plan of Correction: We had varnished it last year
However, we will do it again.
Right now we use one crib.

Standard #: 8VAC20-800-530-B
Description: Based on observation, a crib did not have a tight-fitting bottom cover.
Evidence:
One crib was observed with a loose sheet.

Plan of Correction: Took off crib cover and put
a new fitting one on
