Technical Assistance:
Discussed annual training hours, pressure mounted gates/stairs, refrigerator temperature, choking hazards for children under the age of four, handwashing, menus, emergency drills

Comments:
An unannounced monitoring inspection was conducted beginning at 9:55 a.m. and ending at 12:15 p.m. with the provider. There were 5 children in care ages 2 years old through 4 years old for a total of 10 points. Children were observed during free play, transition time and snack. Childcare areas and supplies were observed.

Three children's records, one provider record , one volunteer record and one household member records were reviewed.

Information gathered during the inspection determined non-compliances with applicable standards or laws and violations were documented on the violation notice issued to the program.

Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it within five (5) calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s)

Standard #: 8VAC20-800-830-C
Description: Based on review of documentation and interview, the provider failed to ensure there was documentation of monthly fire drills and twice annual shelter in place drills.
Evidence:
1.There was no fire drill documented for August, September, October and November of 2023.
2.There was no documentation of at least two shelter in place drills for the 2022 period.

Description: Based on observation, the provider failed to ensure that food considered a potential choking hazard was not served to children under the age for four years of age.
Evidence:
1.On December 12, 2023, the provider was observed preparing snack for children in care: mixed nuts, dried cranberries and blueberries.
2.Three 3 year old children and one 2 year old child were observed being served dried cranberries.
3.Interview revealed that the provider does serve mixed nuts to children under the age of four years of age.

Description: Based on review of documentation, the provider failed to ensure that there was a current one week period menu dated that was either posted for parents to review or provided to parents. Description: Based on observation and use of a digital thermometer, the provider failed to ensure that the refrigerator temperature was maintained at or below 40 degrees Fahrenheit. Evidence: Upstairs in the kitchen, the refrigerator temperature was recorded at 43.5 degrees Fahrenheit.

Description: Based on observation, the provider failed to ensure that balloons were inaccessible to children under the age of five years old. Evidence: On December 12, 2023, there were five children in care ages four years (1), three years (3) and two years (1) old who had access to a blown-up balloon and were observed playing with it. The balloon was observed popping and was still not removed until the Licensing Inspector (LI) intervened.

Description: Based on observation, the provider failed to ensure that pressure mounted gates were not utilized at stair openings. Evidence: In the upstairs of the home there is a pressure mounted gate installed with zip ties at the stair opening. The gate did not fit snugly, did not close completely and was closed with a white child safety lock.
