Comments:
An unannounced monitoring inspection was conducted on 01/31/2024 from 8:10 a.m. to 9:15 a.m. with the provider. There were two children in care, age two-years-old, for a total of four points. Children were observed eating breakfast. Five child records, and one staff record were reviewed. The provider has current certification in CPR and First Aid. No medications were on site and the understands that they must obtain certification in Medication Administration Training (MAT) to give medication. Breakfast was Waffles, syrup, apples, and milk. The first aid kit, flashlight, weather band radio, and fire extinguisher were observed. The emergency drill log, attendance, and menu were also reviewed. Areas of non-compliance are identified in the Violation Notice. Description: Based on review of five children's records, the provider did not ensure that they reviewed with the parent the emergency contact information annually for each child and obtained the parent's signature acknowledging the review.

Evidence: No documented annual review with parent signature were obtained for the following children since the time of their initial enrollment date for Child #1, with a start date of 02/28/22; Child #2, with a start date of 08/22/22; Child #3, with a start date of 08/22/22; Child #4, with a start date of 08/24/20. The standard requires an annual review with parents to ensure emergency contact information is correct.
