Technical Assistance:
Technical assistance the provider is not operating a Family Day Home which requires she operates under the parameters.

Technical assistance on attendane records, menu posting and food bought from home.

Comments:
An unannounced renewal inspection was conducted Feruary 21, 2024 approximately 9:45 am to 11:10 am. There were 12 children for a total of 24 points directly supervised by 3 staff. The home, 3 staff records, 3 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed interative learning activites and water play. There was an assortment of age appropriate materials for children.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please submit plan of corrections within 5 business days in order for it to be included on the website.

Description: Based on observation, the food containers for children were not clearly labeled in a way that identifies the owner.

Evidence:
1. Children lunch containers were observed on a book shelf. The containers were not labeled with child's name.

Plan of Correction: Keep marker and tape by lunch shelves. Keep tape and marker by lunch boxes.

Description: Based on review of the weekly menu, the menu was not kept on file one week at the family day home.

Evidence:
1. Provider did not have a menu posted or available for review for February 20, 2024 and February 21, 2024.

Based on observation of the family day home, the provider did not ensure compliance with these standards and the terms of the current license issued by the department and with relevant federal, state or local laws, and other relevant regulations.

Evidence:
1. Provider does not currently meet the definition of a Family Day Home as she does not reside in the house. This was determined when the Licensing Inspector conducted a walk-through with the assistant caregiver of the home located at 8809 Arlington Boulevard, Fairfax, VA and found no furniture, personal supplies in the bathrooms or clothes in the closet.
2. Provider had a blow-up mattress in a storage bag and comforter in a tote bag in an office room.
3. Provider is operating as a child day center. Provider has not submitted an initial application for a child day center, and she has not received licensing approval to operate a child day center.

Plan of Correction: Not operating as a CDC only have 11 students. Application for CDC will be uploaded to portal by the end of March. Have setup bed.

Description: Based on a review of the written attendance record, the provider did not ensure all children were included on the written attendance record.

Evidence:
1. On February 21, 2024, 3 out of the 12 chldren were not signed in on the atendance record.

Plan of Correction: A teacher will check at 10:00 am to chekc who did not sign in. Talk to parents who repeatedly forget.
