Technical Assistance:
Discussed ensuring that the most recent Family Day Standards (dated July 1, 2021) are available for review and reference.
Provided consultation on standard:
1) 110A: the current form approved by the Virginia Department of Health or a physician's form shall be used to record immunizations received and the results of the required physical examination;
2) 110B: each report shall include the date of the physical examination and the dates immunizations were received and shall be signed by a licensed physician, the physician's designee, or an official of a local health department
3) 170B: the (tuberculosis screening) form shall be signed by a licensed physician, the physician's designee, or an official of a local health department
4) 580C: Daily age-appropriate activities shall include opportunities for one or more regularly scheduled rest or nap periods

Comments:
An unannounced renewal inspection was conducted from 3:30 - 5:00pm with the provider and assistant. There were nine children in care, ranging from 18 months to four years of age, for a total of 20 points. Children were observed playing with a variety of toys. Three child records and three provider/household member records were reviewed along with the daily attendance, emergency drill log, first aid kit and emergency supplies, and required posted information. The provider does not have certification in Medication Administration Training (MAT) so cannot administer medications to children in care.

Standard #: 8VAC20-800-940-A
Description: Based on observation, the temperatures in the refrigerator and freezer compartments were not maintained as required. Evidence: the temperature in the refrigerator was 45 degrees and the temperature in the freezer was 25 degrees.

Standard #: 8VAC20-800-100-A
Description: Based on review of three child records, the provider did not obtain documentation that each child has received a physical examination prior to the child's first attendance or within 30 days after the first day of attendance. Evidence: the record for child #1 (date of first attendance: 2/1/2023) did not have documentation of a physical.

Standard #: 8VAC20-800-180-A
Description: Based on review of three provider/household member records, not all providers/household members had a repeat tuberculosis (TB) test/screening at least every two years from the date of the previous test/screening. Evidence: the record for provider #2 had a TB test/screening dated: 7/8/2019. The record for household member #1 had a TB test/screening dated: 3/1/2021.
