Comments:
An unannounced renewal inspection was conducted on April 15, 2024 between the hours of 10:15 am through 11:20am. There were a total of six children (12 points) observed with two caregivers. A total of five children's files and two staff/household member's files were reviewed. The children were engaged in the following: outside play time, circle time, hand washing, writing, and other organized activities. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 8VAC20-800-830-C
Description: Based on documentation review and interview, documentation was not maintained of emergency evacuation drills.
Evidence:
1. Documentation of the emergency evacuation drills for the months of November and December were not available for review at the time of the inspection.
2. Caregiver #1 confirmed that the emergency evacuation drill was completed, but could not locate the emergency evacuation log sheet .

Plan of Correction: Documentation for the November and December emergency evacuation drills could not be found at the time of the inspection. Emergency evacuation drills were conducted for the months of November and December.

Standard #: 8VAC20-800-70-A
Description: Based on documentation review and interview, parents were not provided in writing required information before the child's first day of attendance.
Evidence:
1. Child #1's record (Start date-04/05/2023) did not contain a completed Information For Parents.
2. Child #2's record (Start date-10/11/2022) did not contain a completed Information For Parents.
3. Child #3's record (Start date-07/31/2023) did not contain a completed Information For Parents.
4. Child #4's record (Start date-07/22/2021) did not contain a completed Information For Parents.
5. Child #5's record (Start date-09/05/2023) did not contain a completed Information For Parents.
6. Caregiver #1 stated that the information required for parents was given verbally, but not in writing.

Plan of Correction: The Information for Parents form will be completed for each child in care and the parents will resign the form to acknowledge receipt.

Standard #: 8VAC20-800-240-A
Description: Based on observation, areas and furnishings of the family day home, inside and outside, were not maintained in a clean, safe, and operable condition.

Evidence:
1. In the basement activity room, there was peeling and chipped paint located on each wall.

Plan of Correction: The room was recently renovated and the walls will be repainted in the next few days. The children were not in allowed in this room, so they were not exposed to the chipped paint.

Standard #: 8VAC20-800-280
Description: Based on observation, potentially poisonous substances, materials and supplies such as, but not limited to cleaning agents, disinfectants, deodorizers,plant care chemicals, pesticides, and petroleum distillates were not stored away in areas inaccessible to children.
Evidence:
1. A bottle of toilet cleaner was located on the floor of the bathroom and accessible to children.

Plan of Correction: The toilet cleaner was placed in a location inaccessible to children at the time of the inspection.
