Technical Assistance:
Technical assistance items discussed during the inspection covered maintenance of the outdoor area in preparation for the summer, and additional batteries needed for the emergency supplies.

Comments:
An unannounced monitoring inspection was conducted today between approximately 9:10 am and 10:45am. There were 8 children (8 points) present with 2 caregivers. The children were observed playing with play-dough, reading stories and building with blocks. The physical plant, attendance logs, evacuation logs, menu, cribs, children's records, caregiver records, household member records, and emergency supplies were reviewed. There were no medications on site. A total of 4 child records and 2 staff records were reviewed.

Violations:
Standard #: 22.1-289.011-F
Description: Based on review of required posted documents and interview with the Provider, the Provider did not ensure that all required documents related to the terms of the license were posted on the premises of the program.
Evidence:
1) On the date of inspection, the findings of the most recent inspection of the facility were not posted. The Provider had posted an Acknowledgement Form instead of the Inspection Report.

Plan of Correction: Moving forward, the Provider shall ensure to post the required documents.

Standard #: 8VAC20-800-480-E
Description: Based on observation, outdoor play equipment was not maintained and free from hazards. Evidence:
A cart on the playground had a broken piece leaving a jagged edge that could result in an injury.

Plan of Correction: Corrected during inspection. The Provider removed the broken part from the toy cart.
