Violations:
Standard #: 8VAC20-800-180-A
Description: Based on record review, and interview with the provider, a current Report of Tuberculosis Screening form, was not obtained every two years from the date of the first screening for each caregiver and adult household member. Evidence: The subsequent tuberculosis screenings for Household member A and Household member B were expired.

Plan of Correction: The TB tests will be redone right away and I will make sure they are done every 2 years in the future.
