An unannounced focused monitoring inspection was conducted on May 1, 2024, from 11:40am to approximately 1pm. There were four children present totaling 10 points and the provider and household member. The Inspectors reviewed the provider?s record and household staff record. Children were observed during free play, diapering and handwashing. Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented. The violation is listed on the violation notice issued to the center and were reviewed with the provider at the exit interview.

Standard #: 22.1-289.035-A Description: REPEAT VIOLATION
Based on record review and interview, it was determined that the center did not ensure that all employees shall undergo a background check every five years.
Evidence:
1. In reviewing staff records, both staff records had expired background checks.
a. Staff #1?s criminal background checks expired on March 20, 2023;
b. Staff #2?s criminal background checks expired on March 29, 2023;
2. The Family Day Home confirmed that the staff member?s criminal background checks were expired as of this year.Plan of Correction: The provider responded with the following: The provider is having a difficult time navigating through fieldprint and online appointments and scheduling to complete this task. Provider will continue to attempt to obtain the criminal background checks successfully as they are expired and are required for the renewal.
