Comments:
An unannounced monitoring inspection was conducted on December 19 2023 between 10:45am and 12:30pm. There were 7 children (19 points) in the care of two assistants. Children were observed hand washing, participating in serving snack, and playing. The emergency plan, assistant records, child records, attendance record and the home was inspected and reviewed. Description: Based on interview and record review, the substitute providers' record did not contain documentation of the time of arrival and departure

Evidence: No documentation was identified that noted when a substitute provider was providing care when the licensed provider exceeded the two hour allotted time per day out of the home.
