Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on 8/13 and 8/24/2020 and concluded on 8/24/2020. The provider was contacted by telephone to initiate the inspection. There were 3 children present and 1 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility

Violations:
Standard #: 22VAC40-665-170-B-17
Description: Based on a review of documentation and interview, the vendor did not have documentation of a written agreement with parents stating that the vendor will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the vendor.
Evidence:
Documentation from two children's records were requested by the inspector and reviewed . The records for child #1 and child #2 did not include documentation of a written agreement with parents stating that the vendor will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the vendor. When asked for this documentation during the inspection, the vendor was unable to provide it.

Plan of Correction: The vendor will have the second page of the children's record model form completed and signed by the parent by the end of the week. Only page one was available for each child at the time of the inspection.
