Comments:

An unannounced renewal inspection was conducted at Families First of Virginia, Inc. on 05/23/2019 from 10:45 am to 4:20 pm and on 05/24/2019 from 10:05 am to 1:25 pm. The agency reports, 39 in care and 36 approved provider homes. During this inspection, four foster child records, four provider home records and one personnel record was reviewed. An additional provider home record was reviewed for background check compliance. Background checks for the board officers were reviewed and are in compliance. An exit interview was conducted on 05/24/2019 at 12:35 pm with the quality assurance supervisor to discuss preliminary inspection findings. An acknowledgement form was signed. There were four citations for violation of the Licensed Child Placing Agencies Standards. Please see the violation notice on the Department's web site for violations of the Standards. Upon the receipt of the violation notice, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: The steps to correct noncompliance with the standard(s); Measures to prevent reoccurrence of noncompliance; Person(s) responsible for implementation and monitoring of preventative measure(s); Date by which noncompliance will be corrected. The licensee will have five calendar days from receipt of the inspection documentation to complete the section titled Plan of Correction, sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days, regardless of whether the Plan of Correction is completed.

Description:	Violation: Based on a review of the records for FC1 and FC2, the licensee failed to include the names of the previous caretakers in the social history. Findings: FC1 was placed with the licensee on 12/28/18 and FC2 was placed on 6/1/18. The records for both children document previous foster home placements. The social history completed by the licensee for FC1 and FC2 fails to include all names of the previous caretakers.

Description:	Violation: Based on review of the record for Foster Child (FC) FC4 and interview with the quality assurance supervisor, the licensee failed to ensure FC4 had at least two face to face contacts for the month of January 2019. Findings: FC4 was placed with the licensee on 8/30/17 and discharged on 2/1/19. The record for FC4 fails to document that FC4 had two face to face contacts for the month of January 2019. During an interview with the quality assurance supervisor, it was acknowledged that FC4 was receiving treatment foster care services and the record failed to document the required monthly contacts for January 2019.

Description:	Violation: Based on a review of the record for FC4 and interview with the quality assurance supervisor, the licensee failed to ensure a written description of each contact made with FC4 was documented in the record. Findings: FC4's record documents that a face to face contact was made in the provider home on 1/2/19. At the time of review, the record for FC4 did not document a written description of the contact made on 1/2/19. During an interview with the quality assurance supervisor, it was acknowledged that the record was out of compliance.

Description:	Violation: Based on a review of the record for FC3 and an interview with the quality assurance supervisor, the licensee failed to include all medical treatment received during the reporting period. Findings: FC3 was placed with the licensee on 8/16/8. The record documents that FC3 had a well child check on 5/17/19. The quarterly progress review covering 2/23/19 to 5/23/19 failed to document the medical treatment received on 5/17/19.
