Comments:

An unannounced, mandated monitoring inspection was completed on January 22, 2020 from 10:00 am to 12:30 pm at Adoption Services located at 3812 Sandalwood Court, Fairfax, Virginia 22031. There have been no new families approved since the last monitoring inspection. Currently the agency is not working with any new birth mothers. During this inspection, three personnel records were reviewed and the policies and procedures and program statement were reviewed. There were two citations for violations of the Standards for Child Placing Agencies. Both were key health and safety standards. The Executive Director was available for the inspection and for the exit interview at the end of the inspection. An Acknowledgement of Inspection form was signed by the Executive Director on the date of the inspection. Please see the violation notice on the Department's web site for violations of the Standards. Upon receipt of the violation notice, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which non-compliance will be corrected. The licensee has ten (10) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days, regardless of whether or not the Plan of Correction has been completed.

Description:	Violation: Based on a review of the personnel record for P-2 and an interview with an agency representative, the agency failed to obtain a search of the central registry and a criminal history record check before three years since the dates of the most recent central registry finding and most recent criminal history record check. Evidence: 1) The criminal history record check and the central registry search results in the record of P-2 were dated 9/28/16 and 10/01/16. This is outside the three year time frame. 2) The agency representative reviewed the file at the time of the inspection and acknowledged that the checks were overdue.
Plan of Correction:	The Executive Director will ensure that the agency obtains a criminal history record check and a search of the central registry for P-2 immediately.
