Technical Assistance:
Technical Assistance provided regarding standard 22VAC40-131-G.3, documentation of training and guidance to foster parents and 22VAC40-131-380, documenting crisis interventions based on concerns presented to caseworkers.

Comments:
This inspection was conducted by licensing staff using an alternative remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 1/04/2021 and concluded on 01/12/2021. The Program Director and Quality Manager was contacted by email to initiate the inspection. There were 41 children in care, and 31 approved provider homes. The agency reports 1 newly hired staff since the last inspection. The inspector emailed the Quality Manager a list of items required to complete the inspection. The inspector reviewed 4 children?s records, 4 provider home records, board officer records (background records only) and 1 staff record submitted by the agency to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the agency. Any violations can be found on the violation notice. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of correction," sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public website within five business days, regardless of whether or not the Plan of Correction is completed.

Standard #: 22VAC40-131-290-K
Description: Based on the review of the foster home provider record for foster home (FH) #4 and based on an interview with agency representatives (AR), the licensee failed to document a tuberculosis screening or test for other household members (OHM) #4 and (OHM) #5.

Findings:
1. At the time of the inspection, there was no tuberculosis screening or test presented for review in the records provided by the licensee for OHM#4 and OHM#5.
2. During the exit interview, AR#1 reviewed the record for FH#4 and was not able to locate a tuberculosis screening or test for OHM#4 and OHM#5. During the interview, AR#1 acknowledged the absence of evidence of compliance in this area.
