Type of inspection: Renewal Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 6/2/2022 from 10:15a.m. to 12:30 p.m., 6/7/2022 from 10am to 3pm, and 6/8/2022 from 10am to 11am. Number of children in care: 32 Number of approved provider homes: 34 The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of children?s records reviewed: (4) and (1) discharged child record. Number of provider home records reviewed and (4) foster home records. Number of staff records reviewed: 6 Observations by licensing inspector: positing of the license and inspection documents was observed. An exit meeting was conducted on 6/8/2022 with the Executive Director and the Quality Assurance Manager. The evidence gathered during the inspection determined (0) violations with applicable standard(s) or law.
