Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 12/12/2023 from 10:00 to 3:00 p.m. The Licensing Inspector met with the Program Manager and the Quality Assurance staff during the entrance and concluded an exit with both. Number of approved provider homes: (26) Number of children in care: (33) Number of children?s records reviewed: (2) and (1) discharge record. Number of provider home records reviewed: (3). Number of staff records reviewed: (3). Observations by licensing inspector: posting of the license and inspection documents was observed. An exit meeting was conducted on 12/12/2023. Technical assistance was provided regarding orientation for new staff and background check sworn statement for foster parent applicants. The evidence gathered during the inspection determined (1) violations with applicable standard(s) or law. Description:	Violation: Based on the record review for PH1 and PH2, the agency failed to notify the applicant in writing of the decision of approval or disapproval within seven business days. Findings: 1. The applicant PH1 was approved on 12/23/22 and PH2 was approved on 7/12/2023. The licensee did not issue a letter in writing of the approval to the applicants. 2. The findings were discussed during the preliminary findings and exit review.
