Comments:

An unannounced monitoring inspection was conducted on May 30, 2019 from 9:30 to 2:15 p.m. Two personnel records and five volunteer files, to include background investigation information, were reviewed. Five residents were in care. Three residents records were reviewed: two current and one discharged resident record. Other documentation reviewed at this inspection included but was not limited to the daily log, case management notes, emergency drill documentation and several medication administration records. Interior and exterior physical plant inspections were conducted. Interviews were conducted with a staff and a resident. Exit meeting was held with Staff S1 to discuss preliminary inspection findings before leaving the facility on May 30, 2019. An acknowledgement form was signed. Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return to the Licensing Office within 5 business days from receipt of the inspection documentation. You will need to specify how the deficient practice will be or has been corrected (merely writing the word corrected is not acceptable). Your plan of correction must contain the steps to correct the noncompliance with the standard(s), the measures to prevent the noncompliance from occurring again; and the position responsible for implementing each step and/or monitoring any preventive measure(s). The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days, regardless of whether the Plan of Correction is completed

Standard #:	22VAC40-100-80-B
Description:	Violation: Based on review of the personnel record for Staff S2 and an interview with Staff S1, the background investigation requirements were not completed prior to employment, as required by the Code of Virginia ?63.2-1726, effective 02/21/19. Findings: 1. The Child Protective Services Central Registry search was not completed prior to employment for S2. 2. Interview with S1 confirmed that the CPS result was received after S2's date of employment.
Plan of Correction:	In order to prevent noncompliance in the future, Miller Home staff will take the following steps: Program Director and Office Manager will coordinate background check completion as early as possible in the interview process. Program Director will not offer a staff position to a potential candidate until all background checks are completed and received by Miller Home staff. Program Director will ensure all background checks are received and in compliance with standards prior to determining a start date with a potential employee.
