Comments:

An unannounced monitoring inspection was conducted on March 30, 2022 from 12:00 p.m. to 3:14 p.m. Six (6) residents were in care. Two (2) residents records were reviewed: one current resident and one discharged resident. One (1) personnel record and one (1) certified volunteer (intern) record, to include background investigation information, were reviewed. Other documentation reviewed at this inspection included but was not limited to the daily log, case management notes, emergency drill documentation, medications, and several medication administration records. Interior and exterior physical plant inspections were conducted. The Program Director was present and available to respond to questions during the inspection. An exit meeting was conducted with the Program Director on March 30, 2022, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. An acknowledgement form was signed. Information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
