Comments:
An unannounced renewal inspection was conducted by two licensing specialists on June 5, 2019 from 11:16 a.m. to 4:46 p.m. and on June 6, 2019 from 9:30 a.m. to 4:24 p.m. Four personnel records were reviewed. 95 residents were in care. Three current residents and two discharged resident's records were reviewed. Other documentation reviewed at this inspection included but was not limited to the daily communication log, movement logs, case management notes, policies and procedures, emergency drill documentation and several medication administration record. Interior physical plant inspection was conducted of one of the houses. Staff S4, S5, and S6 were present and available to respond to questions during the inspection. One resident was interviewed. A meeting was held with the Executive Director, Vice President of Programs, Associate Vice President of Program Services, Director of Quality Assurance, Quality Assurance Coordinator, Assistant Director of UIM Counseling Services and Case Management, Director of Residential, Assistant Director of Residential, and two House Coordinators to discuss preliminary inspection findings before leaving the facility on June 6, 2019. An acknowledgement form was signed at this meeting. Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return to the Licensing Office within 5 business days from receipt of the inspection documentation. You will need to specify how the deficient practice will be or has been corrected (merely writing the word corrected is not acceptable). Your plan of correction must contain the steps to correct the noncompliance with the standard(s), the measures to prevent the noncompliance from occurring again; and the position responsible for implementing each step and/or monitoring any preventive measure(s). The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days, regardless of whether the Plan of Correction is completed.

Standard #: 22VAC40-151-300-B-3
Description: Violation: Based on review of the personnel record for Staff S1 and interview with Staff S7, S1 was hired for a case manager position but did not possess the qualifications required for the position. Findings: 1. S1 has a baccalaureate degree with three years of volunteer experience working with children, however, this regulation requires professional experience working with children. 2. S7 confirmed that the experience documented in the resume was volunteer experience.

Plan of Correction: HR and Payroll Director will communicate to all hiring managers and HR employees that any UIM staff hired must have professional working experience (paid) versus volunteer experience going forward.

Description: Violation: Based on review of the residents' records for current residents CR1 and CR2 and interview with Staff S6, the initial objectives and strategies or "initial service plan" was not completed within the timeframe required by this regulation. Findings: 1. The initial objectives and strategies or "initial service plan" was not completed within three business days following admission for CR1 and CR2. 2. Staff S6 confirmed that the reports were not completed within 3 business days following admission.

Plan of Correction: Provider is in process of applying for a variance from DSS in regards to the licensing regulation. The program follows ORR and DSS guidelines.
