Comments:

An unannounced monitoring inspection was conducted on December 18, 2019 from 9:43 AM - 4:54 PM. The facility reports a total of twenty-five (25) children in placement. During this inspection, records were reviewed for three current residents, two discharged residents, and four personnel. Three Medication Administration Records (MAR) were reviewed. A sampling of staff was reviewed against the CRF Employee Matrix. Interviews were conducted with two residents and two staff. A tour of the facility?s exterior and the interior of Ruby Dobson boy's cottage was conducted. Residential environment requirements were assessed. The following was reviewed for the past six months: daily logs, menus, staff work schedules, incident reports, emergency drill documentation, overnight trip logs and documentation, and readily accessible medical information. Annual fire and sanitation inspections were reviewed. An exit interview was conducted at the conclusion of the inspection with the Executive Director and Director of Specialized Services to review preliminary inspection findings. An acknowledgement form was signed. There were no violations. The licensee has five (5) calendar days from receipt of the inspection documentation to complete the section titled Plan of Correction, sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days.
