Comments:

An unannounced monitoring inspection was conducted in response to a self-reported incident received on August 18, 2020 involving interaction between staff and resident. Due to the outbreak of COVID-19, the inspection was conducted in the form of a desk review on various dates between August 19, 2020 and August 26, 2020. Inspection activities included an interview with agency representative AR1, review of documentation including the serious incident report, policies and procedures, portions of the resident's record, and portions of the staff's personnel record. Exit meeting was held with AR1 via telephone on August 26, 2020. The licensee has five (5) calendar days from receipt of the inspection documentation to sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days.
