Technical Assistance:

The facility is encouraged to review documentation as it pertains to ?physical restraint? as noted in the following policy and procedure documents: ?Behavior Interventions? and ?Emergency and Evacuation Procedures.?

Comments:

This inspection was conducted by the licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring non-mandated inspection was conducted on various dates beginning on 7/28/20 and concluding on 8/27/20 to examine the areas of noncompliance identified in the Intensive Plan of Correction (IPOC). The IPOC was issued on 5/20/20. The Program Director was contacted by telephone on 7/28/20 to initiate the inspection. The licensing specialist emailed the Program Director the documentation needed to conduct the inspection. At the time of the inspection, the facility reported the census was one (1) resident. The following is a listing of the activities for this inspection: Four staff were interviewed on various dates beginning on 7/28/20 and concluding on 8/7/20. Policies and procedures applicable to the IPOC were reviewed. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services website.
