Comments:

This inspection was conducted by the licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An announced complaint inspection was conducted on various dates beginning on 9/10/20 and concluding on 10/2/20. The complaint is in regard to the Service Plan/Quarterly Report and Discharge at this children?s residential facility. The Chief Administrative Officer was contacted by telephone on 9/10/20 in regard to this inspection. The Licensing Specialist emailed the Chief Administrative Officer a list of the information needed to conduct the inspection. At the time of the incident, the facility reported the census was five (5) residents. Violations were not cited in relation to this complaint. Therefore, this complaint is determined invalid. The following is a listing of the activities for this inspection: The Licensing Specialist interviewed the Chief Administrative Officer by telephone on 9/10/20, 9/22/20, and 9/25/20. A local agency representative was interviewed on 9/21/20 in regard to a youth placed at this facility. The record for one youth was reviewed. Relevant policies and procedures were reviewed. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services? website within 5 business days.
