Comments:

This inspection was conducted by the licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 5/27/21 and concluded on 6/30/21. The Chief Administrative Officer was contacted by telephone on 5/27/2021 to initiate the inspection. The Chief Administrative Officer reported that the current census was six (6) residents. The Licensing Specialist emailed the Chief Administrative Officer a list of items required to complete the inspection. The following is a listing of the activities for this inspection: Reviewed one current resident record. Reviewed health care and medication administration records. Two personnel records were reviewed. No discrepancies were found with the CRF matrix. The Chief Administrative Officer was interviewed on 6/18/21. Other documentation reviewed during this inspection included, but was not limited to the following: menus, emergency drill documentation, staff work schedules, and daily log book entries. Description:	Violation: Based upon review of current resident?s, CR1?s, quarterly review and interview with staff, the facility failed to report the resident?s progress towards meeting the plan?s objectives. Findings: 1) The quarterly progress report for CR1 is dated for 5/22/21. 2) This particular report includes three goals. Each goal includes the following objectives: a. Goal #1 ? Objective A and Objective B b. Goal #2 ? Objective A and Objective B c. Goal #3 ? Objective A 3) In the section of this report titled ?progress? for Goal #2-Objective B and Goal #3-Objective A, the following is stated for each objective - ?objective continues until completion date.? 4) No information regarding the resident?s progress is included in the report. 5) During the interview with staff, S3, on 6/18/21, the Licensing Specialist advised S3 of the violation. No additional information was provided by S3 to demonstrate compliance.
