Comments:

A monitoring inspection was initiated on December 1, 2021 and concluded on December 15, 2021. The Program Director was contacted by telephone to initiate the inspection. The Program Director reported that the current census was 1 youth. The inspector emailed the Program Director a list of items required to complete the documentation review portion of the inspection. The inspector reviewed two (2) residents' records and two (2) staff records. An exit meeting was conducted with Program Director on December 15, 2021, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law, and a violation was documented on the violation notice issued to the facility.

Description:	Violation: Based on review of the personnel record for Staff S1 and interview with administrator A1, the Tuberculosis (TB) screening assessment was not completed within the required timeframe. Findings: 1. The TB screening assessment was not completed within the required 30 day timeframe at the time of hire for S1. 2. A1 confirmed that the TB screening assessment was older than 30 days.
