Administration Residential Environment Programs and Services

Comments:

An unannounced monitoring inspection was conducted on 3/15/2022 from 11:55 a.m. to 4:55 p.m. The facility reports a total of 10 children in the long term care program and 8 children in the emergency shelter. An entrance conference was conducted with the Executive Director. During this inspection, three (3) current resident, two (2) discharged resident, and three (3) personnel records were reviewed. Six (6) Medication Administration Records (MARs) were reviewed. Environment standards were inspected in Stickleyville girl?s long term care cottage, Ruby Dobson, boy?s long term care cottage, and the administration building. Residents were observed and informal conversation occurred in Ruby Dobson cottage. The grounds and building exteriors were inspected. The following were inspected: daily logs, menus, and staff work schedules. Preliminary findings were reviewed and an acknowledgement form was signed. An exit interview was conducted on March 17, 2022 at 2:55 p.m.by phone with the Executive Director. There were no violations of Standards for Children?s Residential Facilities.
