Comments:
An unannounced monitoring inspection was conducted on May 3, 2022 from 12:29 p.m. to 4:55 p.m., May 4, 2022 from 10:35 a.m. to 4:47 p.m., and May 5, 2022 from 10:04 a.m. to 2:25 p.m. The facility reported eight (8) new hires since the last inspection. Eight (8) personnel records, to include background investigation information, were reviewed: four (4) personnel records were reviewed entirely and background investigations information only was reviewed for the remaining four (4) personnel records. The CRF Employee Matrix was reviewed. Fifty-four (54) residents were in care. Four resident?s records were reviewed. Other documentation reviewed at this inspection included but was not limited to emergency evacuation drills, menus, staff schedules, staff communication log, medication administration records, and case management notes. Interior and exterior physical plant inspections were conducted. Administrator A1 responded to questions during the inspection. A preliminary findings review meeting was conducted with A1 on May 5, 2022 where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The Acknowledgment of Inspection form was signed and left at the facility each day of the inspection. The exit meeting was conducted with A1 on May 5, 2022. Upon the receipt of the violation notice, a plan of correction is requested for the violations. The plan of correction or Description of Action to be Taken should include, as appropriate, the following: the steps to correct noncompliance with the standard(s); measures to prevent re-occurrence of noncompliance; position(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five (5) business days from receipt of the inspection documentation to complete the section titled Plan of Correction, sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the VDSS website within five (5) business days.

Violations:
Standard #: 22VAC40-151-190-B-1
Description: Violation:
Based on review of the personnel record for Staff S2 and interview with administrator A6, the result of the screening assessment documenting the absence of tuberculosis (TB) in a communicable form was not completed within the required timeframe.

Findings:
1. The TB screening assessment was due by 03/14/2022.
2. The TB screening assessment was completed on 04/11/2022.
3. LI interviewed A6 who confirmed that the TB screening assessment was not completed within the timeframe required by the standard.

Plan of Correction: Provider will utilize YFT's on campus clinic to conduct tuberculosis screenings; as needed to maintain compliance with licensing regulations.

Description: Violation:
Based on review of the personnel records for Staff S1 and S2 and interview with Administrator A6, S1 and S2 did not meet the qualifications required by the standards for Child Care Worker.

Findings:
1. Review of S1 and S2?s resumes did not reveal six months experience working with children.
2. LI interviewed A6 who acknowledged the resumes did not document the required work experience.

Plan of Correction: Provider's human resource dept. will implement a two-step process to ensure all applicant resumes meet the minimum standards of a child care worker. YFT's Director of Training & Evaluation will review all resumes and compare them to DSS regulations. Applicants determined to need further review will be submitted to the Director of Human Resources for further review/determination for employment.

Description: Violation:
Based on review of the personnel records for Staff S1 and S2 and interview with administrator A6, S1 and S2 did not meet the minimum qualification requirements required by the job description.

Findings:
1. Review of the resumes for S1 and S2 revealed that the qualifications for S1 and S2 did not meet the minimum qualifications required by the job description.
2. LI interviewed A6 who acknowledged that the qualifications documented in the resumes for S1 and S2 did not meet the minimum qualifications required by the job description.

Plan of Correction: Provider will review current job descriptions to ensure employees meet requirements listed.
