Type of inspection: Renewal Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 8/23/2022 from 11:50 a.m. to 5:00 p.m. and 8/24/2022 from 9:00 a.m. to 11:00 a.m. The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Number of residents in care at the beginning of the inspection: 15 The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Number of resident?s records reviewed: 4 Number of staff records reviewed: 4 Number of interviews conducted with residents: 1 Number of interviews conducted with staff: 2 Additional Comments/Discussion: The IVE Matrix, staff schedules, menus, insurance documents, two (2) medication administration records, medication storage and labeling, vehicle, and documentation of evacuation drills were reviewed/inspected. An exit meeting was conducted with the Executive Director on 8/26/2022 by Google Meet. The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
