Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 9/1/22 from 2:55 PM ? 4:45 PM and 9/7/22 from 9:35 AM ? 3:33 PM The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. A self-reported incident was received by VDSS Division of Licensing on 8/10/22 regarding allegations in the area(s) of: Suspected Child Abuse and Neglect. Number of residents in care at the beginning of the inspection: 8 Number of resident?s records reviewed: 1 Number of staff records reviewed: 4 Number of interviews conducted with staff: 6 Additional Comments/Discussion: An entrance conference was held on 9/1/22. Reviewed one discharged resident?s record. Interviews were conducted with staff in order to seek clarification about the incident. Relevant policies and procedures were reviewed. The Director of Group Care Services and the Assistant Director of Group Care Services were available and accessible during the inspection on both dates. An exit meeting was conducted on 9/14/22 to review the inspection findings. The evidence gathered during the investigation did not support the self-report of non-compliance with standard(s) or law.
