Type of inspection: Complaint Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 4/8/22 from 1:04 PM-6:09 PM, 5/20/22 from 4:18 PM ? 6:50 PM, and 11/4/22 from 1:44 PM ? 8:37 PM The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. A complaint was received by VDSS Division of Licensing on 3/1/22 regarding allegations in the area(s) of: Structured Program of Care and Prohibitions. Number of residents in care at the beginning of the inspection: 1 Number of resident?s records reviewed: 1 Number of staff records reviewed: 1 Number of interviews conducted with residents: 1 Number of interviews conducted with staff: 3 Additional Comments/Discussion: An entrance meeting was held on 4/8/22. The Program Director was accessible and available during the inspection. The following is a listing of the activities for this inspection: The record for a discharged resident was reviewed. The personnel record for the staff member named in the complaint was reviewed. The daily log was reviewed. A resident was interviewed on 4/8/22. A Residential Relief Counselor was interviewed by phone on 4/14/22. The following staff members were interviewed on 11/4/22: the staff member named in the complaint and the Program Director. An exit meeting was conducted on 2/3/23 to review the inspection findings. The evidence gathered during the investigation did not support the allegation(s) of non-compliance with standard(s) or law.
