Comments:

Type of inspection: Initial On 12/22/23, licensing inspector emailed a letter to provider outlining additional documentation and updates needed following a review of providers initial application packet. Licensing Inspector emailed a letter to provider on 2/8/24 which outlined policies and procedures that were reviewed and select policies requiring additional information to align with VDSS requirements and standards. A tour and physical plant inspection of the residential home being used for a CRF program was conducted on 2/16/24 with the CEO and program Director present during the entire onsite. During the onsite a review of employee records, agency forms, select background checks, sworn statements, fire/health inspections, liability insurances and other required documents for CRF program were reviewed. Licensing inspector on 3/9/24 received and reviewed updated background checks, Central Registry results, and revisions of program forms, policies for compliance. Additional Comments/Discussion: Youth Empowered Solutions is an initial Children's Residential Facility in the Tidewater area of Virginia.
