This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and a virtual tour of the program.
A renewal inspection was initiated on 8/19/2021 and concluded on 8/19/2021. The director was contacted by telephone to initiate the inspection. There were 87 children present and 14 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed five children's records, five staff records, all board officer and agent records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-185-130-A Description: Based on record review and discussion with center staff, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
1. Documentation in the record for Child #2 contained a note from the healthcare professional, which stated that the child was "behind on vaccines." The child had already been attending the center.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22VAC40-185-70-A Description: Based on record review and discussion with center staff, the center failed to obtain documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering.
