This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on August 19th, 2021. There were no children present with 3 staff members. The inspector reviewed compliance in the areas of administration, physical plant, staffing, programming, medication, special care and emergencies and nutrition. No children's records were reviewed and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-185-240-A Description: Based on a review of staff records, the facility failed to obtain the Recognizing Child Abuse and Neglect and the legal requirements for reporting suspected child abuse as require by the Code of Virginia by the end of the first day of assuming job responsibilities.
Evidence:
Staff #1, #2, #3, #4 and #5's records do not contain documentation of Recognizing Child Abuse and Neglect training.Plan of Correction: We will ensure that staff will have Abuse and Neglect training by the end of the first day of employment.
Standard #: 22VAC40-185-540-E Description: Based on a review of non medical emergency supplies, the center did not have a working flashlight.
Evidence:
Center did not have a working flashlight, the flashlight did not have batteries.Plan of Correction: We will ensure that the first aid kits will contain all required documents.
Standard #: 22VAC40-185-550-A Description: Based on a review of documents, it was determined that the center did not have documentation of emergency preparedness plan available for viewing on the day of the inspection.
Evidence:
There is no complete emergency preparedness plan available for viewing on site.Plan of Correction: We will post the Emergency Plan.
Standard #: 22VAC40-185-550-D Description: Based on a review of evacuation drill log, it was determined that the facility did not complete the required monthly practice evacuation drills or two shelter in place practice drills annually.
Evidence:
No evacuation drills were documented for February, April, May and June 2021. There was only one SIP drill held 9/29/2020Plan of Correction: We will conduct evacuation drills monthly practice drilss and we will conduct two shelter in place practice drills annually.
Standard #: 22VAC40-191-40-D-1-C Description: Based on a review of staff records, the facility failed to ensure that employees resubmit Sworn statement of affirmation every five years.
Evidence:
Staff #3's record had documentation of a Sworn statement dated 8/20/2013.Plan of Correction: We will make sure that all background forms are updated every five years.
Standard #: 22VAC40-191-60-C-2 Description: Based on a review of staff records, the facility failed to obtain a central registry finding within 30 days of employment.
Evidence:
Staff #1's record does not contain documentation of a central registry finding within 30 days of employment.Plan of Correction: I will ensure that all staff members have CPS registry within 30 days of employment.
