An unannounced monitoring inspection was conducted on 01/13/2020 from 2:45 P.M.-4:30 P.M. There was one child in care with the provider upon arrival an additional four children came in afterschool for a total of five points. The records for six children, one provider, evacuation drills, emergency plan, first aid supplies, annual training hours, and required posted information were reviewed. Children were observed during naptime, waking up from naptime, arrival of afterschool children, departure, handwashing, free choice playtime, and snacktime. The exceptions to compliance are noted on the violation notice.

Standard #: 22VAC40-111-100-A Description: Based on review of children files, the provider failed to obtain a physical examination within 30 days after the first day of attendance.
Evidence:
1. Child #1's file did not contain a physical examination. Date of enrollment for Child #1 was 09/02/2019.
2. Child #2's file did not contain a physical examination. Date of enrollment for Child #2 was 07/28/2019.
3. The provider verified that the files did not contain documentation of a physical examination.Plan of Correction: The provider has contacted the parents and requested a copy of the physical exam.
