An unannounced monitoring inspection was conducted on 9/9/19 from 10:30am - 12:30pm. During the inspection there were 61 school age children in care with seven staff. Children were observed participating in various activities in the classrooms and gym, and eating lunch. Records were reviewed for six children and six staff during the inspection. Medication, emergency procedures, and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Standard #: 22VAC40-185-140-A Description: Based on a review of six children's records, it was determined that the facility did not ensure that each child in attendance had a completed physical within one month of attendance.
Evidence:
1. The record for child #2 (date of enrollment 9/3/19) did not contain a physical examination.
2. Staff #7 confirmed that the record for child #2 did not contain a physical examination, and had been enrolled for more than 30 days.Plan of Correction: The facility responded: We will have the parents of child #2 obtain a current physical examination. All newly enrolled children will have their records reviewed prior to starting care.
Standard #: 22VAC40-185-510-N Description: Based on a review of the medication being stored at the center, it was determined that the licensee did not ensure that when an authorization expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization.
Evidence:
1. The medication authorization for child #7 expired on 6/30/19, but the medication (Pro Air inhaler and Epi-pen) were still at the center.
2.The medication authorization for child #8 expired on 8/16/19, but the medication (Benadryl and Epi-pen) were still at the center.
3. Staff #7 confirmed that the medication for child #7 and child #8 had been present in the center for more than 14 days since the expiration of the written medication authorization for each child.Plan of Correction: The facility responded: Both children only attending during our summer program. We will contact the parents of each child and asked them to come get their medication. Going forward all medication will be given to the parents when the medication authorization expires. If medication is not picked up after 14 days, then it will be disposed of properly.
