This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program A monitoring inspection was initiated on 12/10/21 and concluded on 12/17/21. The provider was contacted by telephone to initiate the inspection. There were 88 children present and 11 staff present. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 7 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-770-60-B Description: Based on a review of 7 staff records, it was determined that the facility did not ensure that the facility did not ensure that an employee must not be employed until a sworn statement or affirmation has been completed.
Evidence:
1. The record for staff #7 (date of hire 6/18/21) contained a sworn statement or affirmation that was dated 12/15/21.
2. Staff #1 (Program Director) reviewed the record for staff #7 and confirmed the sworn statement or affirmation was completed after employment began..Plan of Correction: The facility responded: All new staff will complete a sworn statement or affirmation prior to beginning employment.
Standard #: 8VAC20-770-60-C-2 Description: Based on a review of 7 staff records, it was determined that the facility did not deny continued employment of a staff who did not have a search of the central registry finding within 30 days of employment.
Evidence:
1. The record for staff #4 (date of hire (10/29/21) did not contain documentation of a completed search of the central registry finding.
2. The record for staff #7 (date of hire (6/18/21) did not contain documentation of a completed search of the central registry finding.
3. Staff #1 (Program Director) reviewed the records for the staff #4 and staff #7, and confirmed that the search of the central registry finding has not been received for either staff , and that both staff had been employed for more than 30 days.Plan of Correction: The facility responded: Staff #4 and staff #7 has previously completed the search of the central registry, but we were unable to locate it for viewing during the inspection. If it can't be located staff #4and staff #7 will complete a new search of the central registry. All new staff will complete a search of the central registry upon hire and we will ensure it is received within 30 days.
Standard #: 8VAC20-780-160-A-1 Description: Based on a review of 7 staff records, it was determined that the facility did not ensure that all staff provide documentation of a negative TB screening at the time of employment and prior to coming into contact with children.
Evidence:
1. The record for staff #7 (date of hire 6/18/21) did not contain documentation of a negative TB screening.
2. Staff #8 (Program Director) confirmed that the both listed above did not have a negative TB screening available for viewing during the inspection.Plan of Correction: The facility responded: Staff #7 will be sent to complete a TB screening. All new staff will complete a TB screening prior to employment.
