Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:30 am - 11:45 pm. There were twenty four children in care: four infants with one staff, seven toddlers with two staff and thirteen preschoolers with two staff. One additional staff was also present. The children were observed at circle time, listening to music, playing on the playground and in group activities. Five children's records were reviewed. Standard #: 22VAC40-185-330-B
Description: Based on observation and measuring, the center failed to maintain the resilient surfacing on the playground at the required depth of 6 inches.
Evidence:
1. The inspector observed the playground and measured the resilient surfacing (bark mulch). There was approximately 2 to 3 inches of mulch on both sides of the slide and on the back side of the climbing equipment. Fall height of the climber is approximately 48 inches.

Plan of Correction: The teacher will rake the additional mulch from under the slides and under the ramp into the areas. The playground will be monitored to ensure the resilient surfacing remains at the required depth.
